• Evaluated and provided interventions for 30 patients with the following diagnoses: ABK amputation, chordoma removal at C5 and C6, CVA of different severities, MS of different severities,  bilateral knee replacements, Parkinson’s, falls with multiple injuries, myasthenia gravis, spinal stenosis, aphasia, hemarthrosis, CHF, AMS, injuries from MVA, dementia, low vision, and UTI

• Wrote up to 100% of all patients’ documentation: Fieldwork educator commented “through documentation with focus on details”

• Completed 6 sets of CAHAI, PSFS, and 9-hole peg to document patients with CVA at baseline and discharge

• Used MoCA, including version 2, on 10 patients

• Increased CAHAI scores of patients with CVA up to full scores

• Increased average PSFS scores of 2 patients with CVA by 3 and more points 

• Discharged 33% of the patients treated (11/30) at Modified Independent

• Discharged 33% of the patients treated (11/30) at Supervision to Minimum Assist

• Completed Total Assist transfers for patients, who weighs 125.4lb and 230lb, with occasional assistance 

• Applied evidence-based practice, strategy training, on patient with reduced attention and increased impulsivity, and discharged patient at Modified Independence level

• Presented strategy training to 25 therapists, including physical and occupational therapists, and witnessed own fieldwork educator using the therapeutic technique on her patient

• Developed and presented therapeutic invention tools to diversify therapeutic interventions to 15 staff members, including physical and occupational therapists. Therapists commented the invention as “creative and refreshing

• Compiled and introduced evidence-based treatment notebook, including documentation and evaluation checklist, for both new occupational therapists and occupational therapy students: An incoming occupational therapist asked to lend her the notebook to improve her documentation and evaluation

Achievements from treating patients with CVA

• Conducted 61 evaluations, including home situation assessments, ADL/IADL assessments, objective measures (goniometry and strength), vision screening, sensation screening, CAHAI, CMA, PSFS, and MAS

• Increased patients’ CMA stages: Successfully assisted patient to transition from stage 2 to 4 for both shoulders and arms

• Increased patients’ CAHAI score by 200%

• Increased patients’ MAL score by 200 to 500%

• Increased patient’ PSFS score by 400%

• Created tools depending on the needs of the patient: zipper stabilizer, theraband fishing reel, string ball toss, and thick line guides

• Recommended AD to increase speed of cutting food from 90 to 7 seconds

• Present an in-service session on the strategies of kinesiotaping to reduce shoulder subluxation for patients with CVA

 

Achievements from treating patients with chronic pain and fatigue

• Conducted 16 pain evaluations, including general pain assessments (including all aspects of occupations) and PSFS

• Increased patients’ PSFS score by 200%
